The 2023-2024 membership packet includes the following membership renewal documents for you to complete:

1. Application

2. Parent Code of conduct

3. Waiver & Release of Liability

4. Parental Consent & Indemnification Agreement

5. Photo Release Form

Please be sure to include on the application the USFSA # of any applicant currently registered with USFSA and renewing their membership or transferring from another club.
